 note = {P(論文), 慢性腎臓病（CKD）は長年の間に徐々に腎機能が低下する、さまざまな腎臓病の総称である。自覚症状がないまま進行し、腎機能が低下してしまうCKDも多い。CKDを発見するには、尿検査と血液検査を行い、①タンパク尿、血尿などの検尿異常、②糸し球きゅう体たい濾ろか過量りょう（GFR）＜60mL/分/1.73㎡のいずれか、または両者が持続する場合、CKDと診断される。
　CKDは糖尿病腎症、IgA腎症、持続する高血圧、そして遺伝疾患である多発性嚢のうほうじん胞腎などによって引き起こされる。CKDの原疾患を調べるには、まず腎超音波検査などの画像検査を行うが、腎病理組織所見を観察するために腎生検が必要になる場合もある。CKDは透析療法や腎移植を必要とする末期腎不全（ESKD）のリスク要因である。さらにCKDは脳卒中や心臓病、末まっ梢しょう血管病などの心血管病（CVD）の強力なリスク要因となる。
　新たな重症度分類では、CKDはステージ1からステージ5（ESKD）までのGFRのステージ、およびアルブミン尿・タンパク尿の程度によって分類される。これはアルブミン尿・タンパク尿の程度が、GFRとともに重要なCKDの予後に関連する因子であるからである。早期にCKDを発見し、評価し、治療を行えば、ESKDへの進行を遅延させ、さらにCVD発症を抑制できる可能性がある。, Chronic kidney disease（CKD）is a general term for kidney diseases showing progressive loss in renal function over a period of years. Most people may not have any symptoms until their CKD is advanced. Urine and blood tests including urine protein or albumin and serum creatinine can detect CKD. Persistent proteinuria means CKD is present. Glomerular filtration rate（GFR）is the best estimate of kidney function，and estimated GFR（eGFR）below 60mL/min/1.73㎡ also means CKD is present.
　CKD may be caused by diabetes，glomerulonephritis，such as IgA nephropathy，persistent hypertension（high blood pressure），and inherited diseases，such as polycystic kidney disease. To investigate the underlying cause of CKD，various forms of medical imaging，such as ultrasonography，CT and MRI and often renal biopsy are employed.
　CKD may progress，eventually leading to end stage kidney disease（ESKD），which requires dialysis or a kidney transplant to maintain life. Also，CKD is an increased risk factor for cardiovascular disease（CVD），such as stroke，heart disease and blood vessel disease.
　Recent guidelines classify the severity of CKD by GFR（from G1 to G5）and the degree of albuminuria or proteinuria. Early detection, evaluation and treatment of CKD can help to slow or prevent its progression to ESKD and the development of CVD.},
